Easter Seals NH Military & Veterans Services (ESNH-MVS) provides Care Coordination to Service Members, Veterans, and Families (SMVF) who live or serve in New Hampshire. All who serve or have served in the military – plus their families – are eligible for free, confidential, local assistance, regardless of VA eligibility. Care Coordination services may include crisis intervention and emergency assistance, assessment, care planning, service coordination, and case management, including (but not limited to) specialized programs that address housing/homelessness, employment, substance abuse, aging veterans, and justice-involved veterans. The program works in close collaboration in the statewide provider system, and also provides resource/referral assistance to SMVF and other service providers.
Emergency financial assistance is available through Veterans Count as a “hand-up” in the context of overall services that promote self-sufficiency. Veterans Count is a philanthropic program of Easter Seals. The Care Coordination services are funded through a combination of public funding (state/federal government contracts and subcontracts) and private philanthropic funds raised by Veterans Count.

As a leader in social services for individuals with disabilities and special needs, Easterseals NH employs more than 1,400 individuals who serve more than 26,900 children, adults and seniors in over 90 programs throughout the state.
In addition, Easterseals NH includes Maine and Vermont. It is recognized as one of the most progressive and diverse Easterseals nationally.
